Appointment Checklist
- This form is a request for an appointment, and not the final appointment
- We will call you very soon to finalize your appointment
- If you need an urgent appointment – please call our office at 919-844-4344
- We require current patients to fill out a new demographics form once in a year.
- You may fill out the form prior to your visit and bring it with you.
Appointment Cancellation
- Unless canceled at least 24 hours in advance, a fee of $50 may be charged for no shows.
- There will be a fee of $50 for any checks returned by the bank.
Triage Protocol
- Our clinic gives our existing patients an option to walk-in during regular business hours. We reserve a minimum of one slot per hour for patients who prefer to walk-in for their immediate needs. Our goal is simply to avoid unnecessary visits to the emergency room. Should there a need after business hours, please call 919.844.4344 and your call will be automatically transferred to our auto-attendant system. We always have a provider on call 24/7.
